Important Information - please read before completing the form

Create a username: before we can register an additional administrator profile to your firm account, your new administrator will firstly need to create a username. The username must be 8 to 10 characters long. It can include letters and numbers, but no spaces or special characters.
Complete the form: once you have created a new username for the setup of an administrator profile, complete this form.
Await confirmation from Zurich: it is essential to wait for email confirmation from Zurich that the new username has been assigned to an administrator profile before logging in to the Life Protection Platform with the new username. Confirmation usually takes two working days.
